Comprehensive Care Program for CFSM
The Comprehensive Care Program for CFSM provides additional support for cystic fibrosis patients by helping reduce the cost of therapy while providing better quality of care.
Who is eligible?
- Any cystic fibrosis patient or Shwachman-Diamond Syndrome patient in the U.S. or Puerto Rico who takes ULTRASE® or ULTRASE® MT
With a prescription of ULTRASE® / ULTRASE® MT Enteric-Coated Microspheres or Minitablets, what will the patient receive?
- Up to $20 for out-of-pocket expense for a prescription of ULTRASE® or ULTRASE® MT
- SCANDISHAKE® (24 envelopes) or SCANDICAL® (two 8-ounce canisters)
- ADEKs® Tablets (60-count) or Pediatric Drops (one 60mL bottle)
- Certificate for FLUTTER® mucus clearance device, redeemable with physician approval
- Cystic Fibrosis: A Guide for Patient and Family educational book
How does the Program work?
- The Program is available to patients with an ULTRASE® or ULTRASE® MT prescription only.
- The patient should call the Comprehensive Care for CFSM Program Line at (866) AXCAN-RX (866-292-2679) to enroll.
- After filling the prescription, the patient should mail a proof-of-purchase for the allotted supply of ULTRASE® or ULTRASE® MT and the Program Enrollment Form to:

- Program participants will receive the allotted supply of
- - SCANDISHAKE® (24 envelopes) or SCANDICAL® (two 8-ounce canisters)
- ADEKs® Tablets (60-count) or Pediatric Drops (one 60mL bottle) - After enrolling in the Program, participants are eligible to receive a certificate for a FLUTTER® mucus clearance device, if prescribed by a physician.
- New participants will also receive a copy of the educational book, Cystic Fibrosis: A Guide for Patient and Family, by Dr. David M. Orenstein.
